Basic Life and AD&D – Lincoln Financial Group
Open Sky Wilderness provides you Basic Life and AD&D insurance that is 100% employer paid.
Your benefit is 1 times your annual salary. Benefits reduce to 65% of original amount at age 65, and 50%
of original amount at age 70.
Long Term Disability – Lincoln Financial Group
Open Sky Wilderness provides you Long Term Disability insurance that is 100% employer paid.
Your benefit begins after 90 calendar days of disability caused by the same or a related sickness or injury,
which must be accumulated within a 180 calendar day period. You will receive a monthly benefit of 60%
of your basic monthly earnings up to $6,000 maximum.
Short Term Disability – Lincoln Financial Group
Open Sky Wilderness offers you Short Term Disability insurance that is 100% employee paid.

Your benefit begins on the 15 consecutive day of disability due to accidental injury or sickness. You will
receive a weekly benefit of 60% of your basic weekly earnings up to $1,000 maximum.
Hospital Indemnity – Reliance Standard
Open Sky Wilderness offers you Hospital Indemnity insurance that is 100% employee paid. Hospital
indemnity insurance will pay you a cash benefit to help cover some of the medical costs leftover from
your primary insurance plan. The amount of payment is dependent on the condition for which you are
hospitalized and how long your confinement lasts. See additional plan summary for details.
Group Accident – Reliance Standard
Open Sky Wilderness offers you Accident insurance that is 100% employee paid. Accident insurance will
pay you a cash benefit when you have a minor accident. The amount you receive is dependent on the
type and severity of injury, and the type of treatment required. See additional plan summary for details.
Group Critical Illness – Reliance Standard
Open Sky Wilderness offers you Critical Illness insurance that is 100% employee paid. Critical Illness
insurance pays you a cash benefit to use towards medical bills, ongoing treatment, or personal expenses.
The amount of payment is dependent on the condition for which you are diagnosed and the treatment
