The Benefits of Working at Identity MSO
At Identity MSO we recognize that success is dependent on our team members’ commitment to excellence. In
acknowledgement and appreciation of this contribution, we are pleased to offer a competitive total compensation
package that consists of salary, bonuses, paid time-off, and a complete benefits package.
2021 Benefits
Each year Identity MSO holds an open enrollment. At that time, you can choose the following:
• Change the benefit plans you are enrolled in
• Change who you cover by adding or deleting dependents
What you need to do:
To make sure you have the coverage you want:
• Read this brochure to see the highlights
• Discuss your benefit needs with your family
• Decide what benefits you want for the 2021 plan year
Who is Eligible?
You are eligible for the benefits program upon meeting the following criteria:
• A full-time employee who is regularly scheduled to work at least 30 hours each week
• Coverage is effective first of the month following date of hire.
You may also elect coverage for the following people:
• Spouse
• Dependent children up to the age of 26
Following open enrollment, changes to your benefit elections are only allowed in the case of a qualifying event. The
following is a list of qualifying events:
• Change in marital status
• Birth or Adoption
• Part-time to Full-time
• You or your dependents experience a loss of coverage
You have 30 days from the time of the qualifying event to notify Human Resources of your change.
Paychex Open Enrollment Elections
The Paychex Open Enrollment system will be available from 11/16/2020-11/30/2020. All team members must elect their
benefits for 2021, your benefits will not roll over. Additional documentation must be provided to human resources in
order to validate your dependents.

Company reserves the right to modify, amend, suspend, or terminate any plan at any time, and for any reason without prior
notification. The plans described in this book are governed by insurance contracts and plan documents, which are available for
examination upon request. We have attempted to make the explanations of the plans in this booklet as accurate as possible.
However, should there be a discrepancy between this booklet and the provisions of the insurance contracts or plan documents, the
provisions of the insurance contracts or plan documents will govern. In addition, you should not rely on any oral descriptions of these
plans, since the written descriptions in the insurance contracts or plan documents will always govern.
