Select the type of provider you need (e.g., doctor, facility, mental health).

Enter your preferred location.

Select whether you want to search by provider
specialty or provider name.

Relevant results will be displayed.

Dental / Vision plans

Go to blueshieldca.com/fad and click Dentists or Vision Care

Continue as a guest and enter your home zip code

For Dental PPO choose Dental PPO (Group Plans)

For Dental HMO low or high choose Dental HMO (Individual/Family or Group Plans)
