Additional Benefits

Basic Life/AD&D – Employee Only – Anthem Blue Cross
MarketCast LLC provides basic life insurance at no cost to you. Your life & AD&D benefit amount is 1.5 times
annual earnings up to $500,000. Benefits receive a 35% reduction at age 65 and a 50% reduction at age 70.

Guaranteed issue amount is $500,000; anything above will require an evidence of insurability form.

Voluntary Life/AD&D – Anthem Blue Cross (Voluntary - 100% Employee Paid)

• Employee benefit – in increments of $10,000 up to $500,000 not to exceed 5x annual salary. Evidence of
Insurability needed for amounts over $100,000, if you are a new hire. All amounts are subject to Evidence
of Insurability for exsiting employees.
• Spouse benefit –increments of $5,000 up to $250,000. Evidence of Insurability needed for amounts over
$10,000. All amounts are subject to Evidence of Insurability for late entrants.
• Child benefit (15 days - age 26) – may choose an amount of $1,000, $2,000, $4,000, $5,000, or $10,000.
All amounts are subject to Evidence of Insurability for late entrants.
• Benefits receive a 35% reduction at age 65; 50% reduction at age 70

Long Term Disability – Anthem Blue Cross (100% Employer Paid)
Replaces 60% of your monthly income up to a Max of $15,000/month, after you have been disabled for 8 days.

Short Term Disability – Anthem Blue Cross (100% Employer Paid)
Replaces 67% of your monthly income up to a Max of $3,000/week, after you have been disabled for 180 days.

Employee Assistance Program – Anthem Blue Cross (100% Employer Paid)
Anthem’s Employee Assistance Program (EAP) provides trained professionals to assist you with personal and
workplace issues. The program is voluntary and confidential; only your EAP professional will know you have
called. This program is paid for by Marketcast and also includes assistance for your immediate dependent family
members.

Commuter Benefits – Discovery Benefits (100% Employee Paid)
Discovery Benefits offers commuter benefits including mass transit and parking benefits that save employees up
to 40% on their commute to and from the office. Monthly limit of $270.
