Facility Visits
Facility visits to long-term care facilities is the key activity that ombudsmen in Pennsylvania perform. Facility visits are
conducted are a periodic basis either annually or quarterly depending on the facility type. A Facility Visit form is completed
for each annual or quarterly facility visit.
Prior to attending training, you will shadow a facility visit with an experienced ombudsman. While shadowing that
visit, complete the Trainee Journal for Initial Facility Visit.
These forms are available under the Attachments tab on the left-hand side. Click on the paper clip to view all downloadable
and fillable documents. Once you click on a document in the list you can view, complete, or save the document. When you
are finished viewing, completing, or saving the document, click on the small x in the upper right-hand corner to close the
document and return to the manual.
