AFLAC Supplemental Insurance
Medical expenses don’t always come from an accident or critical illness, sometimes it starts with
a visit to an urgent care clinic because of a sharp pain in the side. While health insurance covers
some of the expenses, you will likely be responsible for a portion of the cost. Aflac is available to

help with those unexpected expenses.

Gray Ohio Valley offers Accident, Disability, Cancer and Life Insurance policies through Aflac with

convenient payroll deduction. All policies are portable and can be continued at the group rate.

Prescription Safety Glasses Reimbursement
To ensure our team members put safety first by wearing the appropriate Personal Protective
Equipment, Gray will reimburse those who work on a job site on a regular basis for prescription

safety glasses (clear lenses only). The safety glasses must meet ANSI Z87 requirements for
prescription safety eyewear. Team members are eligible for reimbursement once every twenty-four
(24) months, up to a maximum reimbursement of $150.
