Support Advisor
With a Home Care Package, you can decide how much involvement you’d
like to have in the management of your package. Your Just Better Care Support Advisor is a health care professional
who works with you (and your carers or representatives) to link you to the services and other resources that you want.
Your Support Advisor will assist you to:
‒ Navigate the system and get the services you need to remain living at home
‒ Plan for the future
‒ Make adjustments if your
circumstances and needs change
Support can include assessment,
care planning, scheduling your visits, managing budgets and reviewing how everything is going. At your first meeting with Just Better Care you can discuss how much assistance you need managing your package and adjust your budget accordingly.
Just Better Care also offers after hours phone support. It’s personal service. Just for you.
A guide to Home Care Packages
Individual budget
How your individual budget is spent will be discussed when you design your care plan. An individual budget provides you with greater transparency, so you’re able to see what funds are available in your package, and how those funds are being spent. All care and services provided to you through a Home Care Package must be able to be paid for within your package budget. If you want additional services, we can arrange for you to pay and top up your package with additional contributions.
Your package budget is made up of:
‒ The Government subsidy
‒ Basic daily fee / income-tested care fee
‒ Top up funding
Just Better Care will provide regular monthly statements that show how your budget is being spent. The statement will show you the income and expenditure under the package, as well as any unspent funds. Any unspent package funds must carry over from month to month, and from year to year, for as long as you continue
to receive care under the package. You can use these funds to access additional services as you require them.
