Dependent Life Choice of $5,000 increments for spouse/domestic partner.
Insurance Not to exceed 50% of employee elected amount

$5,000 or $10,000 - Child (6 months – 26yrs)

Reduction Schedule 35% at Age 70
Additional 20% of original amount at age 75
Benefits terminate at retirement

Accelerated Death You can access a portion of your life insurance while you
Benefit are still living if you are diagnosed with terminal illness
which results in an expected life span of 12 months or less.
Portability If coverage has been in force for at least 12 months, you
may continue coverage for a specified period of time after
your employment by paying the required premium. A
written application must be made within 31 days of your
termination.
Conversion Privilege If any or all of your group life insurance ends (for example,
due to termination of employment), you will have the
opportunity to convert some or all of this coverage to an
individual policy.
