        What do I do in case of a medical emergency?
        If  you  believe  you  need  emergency  medical  services,  immediately  dial  911  or  go  to  the
        nearest emergency room.  Notify your primary care physician the next business day or as
        soon as possible.  Your Medical Insurance Member Identification Card should be presented
        at the time of service, and co-payments are due at the time of service.

        What do I do if I want to change my Kaiser Primary Care Physician (PCP)?
        Call the Health Insurance Carrier’s Member Services toll free # listed on your ID card or in
        the benefit summary in this booklet.  A Customer Service Representative will assist you in
        changing your PCP.

        What do I do if I need a new ID card?
        Call the Health Insurance Carrier’s Member Services toll free # listed on your ID card or in
        the benefit summary in this booklet.  A Customer Service Representative will assist you.
        Make sure to verify your mailing address with the Carrier.

        When will I receive my new ID card?

        ID cards are mailed to the member’s home within seven to ten (7-10) business days after
        being  enrolled.    New  ID  Cards  are  also  mailed  whenever  a  change  occurs  that  affects
        information listed on the card.

        What do I need to do if I need to see a specialist?
        Kaiser  –  Your  PCP  will  refer  you  when  you  require  services  from  a  specialist.    Female
        members can self-refer for their annual well women visit as long as the specialist is within
        the same medical group as your PCP.
        PPO – You may make an appointment directly with a contracting specialist.  No referral is
        required.
