Glossary

Initial Eligibility When you first become eligible to participate in our

benefit program
Open Enrollment An annual event that allows an employee to enroll or
change coverage for themselves or eligible
dependents.

Formulary A published list of the prescription drugs covered by
the insurance carrier.

Non-Formulary Prescription drugs that are not listed on the formulary
list. These drugs may be available at a higher
copayment to the member.

Generic A drug that is produced and sold under the chemical
name. These drugs are considered therapeutically
equivalent to the brand name. Generic drugs, when

available, are usually the best value.
Brand Name A drug that is produced and sold under the original
manufacturer’s brand name.
