How do I add or delete family members?
You will need to complete a membership change form which can be obtained from your
Human Resources Administrator. Please note – there are only certain instances when family
members can be added to your plan, such as initial enrollment, marriage, birth or loss of
coverage. In these instances, you typically only have thirty (30) days to add your family
members.
How do I search for a specialist or participating physician?
(1) Call the carrier that is listed on your ID card and a representative will assist you in
locating a physician or a specialist in your area.
(2) You can utilize the carrier’s website and search the directory online.
Can every family member choose their own Primary Care Physician or does
everyone need to have the same physician?
Each family member may select a different personal physician.
If I do not select a Primary Care Physician what will happen?
The carrier will select a Primary Care Physician for you, and you will be notified when you
receive your ID card.
Do I need to carry my ID card with me at all times?
We recommend that you carry your ID Card at all times. You will need to present your ID card
at your doctor’s office or the hospital, this will allow the provider to bill correctly for the
services received.
How often can I change benefit plans?
You may only change benefit plans during your company’s annual open enrollment period.
