A beneficiary designation is a document that allows you to designate who would receive a benefit in the
        event you pass away. Now is the perfect time to ensure your beneficiary designations are up to date.

        Beneficiary designations can save your loved ones’ time, money, and stress at an already difficult time. It is
        important for you to review and update your beneficiary designations whenever you experience major life
        events such as marriage, birth/adoption, or divorce. Complete the forms below to establish or update your
        beneficiaries. Once complete, make a copy for yourself and forward the designation to the appropriate
        agency. Please note, only complete the Nationwide designation form if you are enrolled in deferred comp.

            •  CalPERS: Online at my.calpers.ca.gov, or complete the Pre-Retirement Lump-Sum Beneficiary
               Designation Form and fax or mail it to CalPERS.
            •  MetLife Life Insurance: Complete the Group Term Life Insurance Beneficiary Designation Form and
               mail it to MetLife Record Keeping Center.
            •  Nationwide Deferred Compensation: Online at mcdc457.com, or complete the Beneficiary Designation
               Form and email it to rpublic@nationwide.com.
