Vulnerable and underserved populations:
The department of Health and Human Services (HHS) describes vulnerable and underserved communities as they may include members of the minority populations or individuals who have experienced health disparities.
The term vulnerable is a person who is in need of special care, support or protection because of age, disability or risk of abuse or neglect. Underserved means under resourced or not having sufficient service.
• Latino population
• African American
• Refugees
• Individuals with limited English proficiency
• New mothers and women with children
• Individuals with disabilities
• Medicaid eligible consumers who are not enrolled in spite of being eligible
