 For carers, we held 18 sessions of ‘A Map Through the Forest’. These sessions aim to enhance their knowledge and provide access to support from others who understand what it means to care for someone with a life-limiting illness.
We have also offered educational opportunities to other health professionals who work with palliative care patients and whānau through various programmes, including the Hospice New Zealand Fundamentals of Palliative Care, and Syringe Driver Competency, Health Care Assistant training, as well as other one-off sessions on a diverse range of topics.
Towards the end of the year, we expanded access to our education sessions, such as Clinical Skills, via the online Teams facility to include other health professionals and hospices.
HOSPITAL PALLIATIVE CARE
Our specialist palliative care consultation service for Hutt Hospital is provided by a team of four health care professionals. The team consists of two Clinical Nurse Specialists, a Nurse Practitioner Intern and a Palliative Medicine Specialist.
Based at Hutt Hospital, the team provides specialist advice and support to the Hospital’s medical teams, nurses and other allied health professionals for patients with complex care needs.
Over the last year, two formal education sessions were organised for the hospital nursing staff, with 40 people attending. These sessions covered an introduction to Palliative Care, through to Advanced Palliative Care.
PRIMARY CARE
Our team of two Specialist Palliative Care Nurses, known as Palliative Care Facilitators, help support, facilitate, and coordinate palliative care services with the 18 primary care practices in the Hutt Valley. The team’s role is to create systems that enable primary care providers to support the palliative care needs of their patients and whānau in the community.

The team works closely with the Te Awakairangi Health Network, which is the local Primary Health Organisation to look at the distinct needs of those working in primary care and understand how Te Omanga Hospice can support them.

At the Palliative Care Nurses NZ Conference in March, Palliative Care Facilitators Anna Garton and Fiona Machin presented on the details of their working practice and the services we offer at Te Omanga Hospice.

This year has been difficult for Primary Care in the Hutt Valley, with most practices closed to new patients and wait times for an appointment extending into weeks. The team has worked hard to support the practices to provide equitable, quality, generalised palliative care to patients and whānau.
