INCIDENT REPORTING FORM
Task: Reporting a work place injury or near miss. 10th September 2018 Brief:
GT Electrical Heath and safety policy can be found on the wall in the workshop at 7 Hurring place, Newlands.
In Summary:
Our Heath and safety policy is a commitment between management and staff. Together we are creating a workplace that allows all team members to safely fulfill their duties without any negative effects to their health.
Our workplaces often includes, homes, schools, office environments, building sites, and other public spaces. The safety of public, other tradesmen and other team members is key our H&S policy.
If you ever see anything that is not safe, SPEAK UP. A safe workplace needs you. If you are injured or experience a near miss please follow the steps below:
It is important that all near misses and injuries are reported to the office as soon as possible. THIS IS COMPORSERY NO MATTER HOW BIG OR SMALL.
We all have a commitment to each other to create the safest work place possible. For this to be possible we need to learn and make changers after ever near miss or injury. Nobody wants a team member to have a serious injury when it could have been prevented by reporting your near miss.

You have a legal obligation to report all workplace injuries and near misses.
We also, as a company, have a legal obligation to report to workSafe any notifiable incidents.
We need all injuries and near misses reported to have a full picture of how efficient our processes and training is.
If you witness someone taking a risk but dont feel like you can say anything direct to them, please speak up to management. We will look into matters sensitively.
