Important forms and agreements
On the following page you will find an explanation of the important forms/agreements and where to find them. You will need to read through these in readiness for step two, the pre-mediation, which is usually conducted on the phone. The mediator will refer to these forms in the first session.
Funding Declaration Form
If parties are eligible for the government subsidy, relevant information will need to be supplied by both parties before the pre-mediation session. The mediator will assess and evidence eligibility and sign the funding declaration form on your behalf. Parties who are not eligible for government funding, and/or have not provided the correct documentation for funding to be approved, must agree to pay their share before mediation services are delivered. You do not need to fill this form in - this is done by the mediator. Even if you’re not eligible for the fully funded service, the cost is subsidised and will never cost more than $450 per person – much less than the cost of filing an application at the Family Court.

Agreement to Mediate Form
This is the formal agreement between the parties undertaking the mediation and the mediator. The agreement outlines all expectations and responsibilities of each party, the mediator and other relevant information such as the mediation process. During the pre-mediation session, the mediator will talk you through this form to gain your agreement to mediate. You do not need to fill this form in - this is done by the mediator.

Compliments and Complaints Form and Evaluation Form
We welcome and appreciate feedback at any stage of your service - whether complementary or otherwise. Your evaluations and any suggestions you have, help us to improve our service delivery so we can serve the best possible needs of our clients.
For the Compliments and Complaints Form please refer to the appendix at the back of this booklet. At the conclusion of your mediation you mediator will give you the Evaluation Form.
