REGISTRATION STATEMENTS/SIGNATURE

I hereby submit my registration to enroll as a student of the True Hope Bible Institute of True Hope
Ministries of America in the Christian Ministry Associates Degree Program.

I have read, understand and agree to the terms and conditions of the Student Handbook in general and
specifically for the Christian Ministry Associates Degree Program and state that I agree to abide by these
terms and conditions and will comply with the requirements thereof.

I understand that the registration deposit is part of the overall tuition and must accompany this registration
form or the enrollment process will not commence until such time as the registration deposit has been paid.
I understand that the registration deposit is not refundable once the Student Enrollment Package has been
placed with the USPS for delivery to my given address and that after 60 days of mailing, the enrollment
forms must be submitted to True Hope Ministries of America or this registration shall become null and
void.
