                       •  Train all staff not to file any reports, including consultation

                          reports or letters, without the physician’s review.

                       •  If the patient cancelled or was a no-show, contact

                          the patient to reiterate the reason for the referral and

                          document this discussion in the medical record.

                       •  Make sure there is a clear understanding as to who is

                          responsible for the patient’s care.



                   Consulting Physician

                   When you receive a request for consultation or transfer of care,

                   take the time to review the records and track the appointment.

                   Your practice should have a standard procedure for referral-
                   related communication which clearly defines everyone’s
                   responsibilities. Consistency and clear instructions will make

                   the process run more smoothly, benefiting both the patient and

                   the practice. While referring physicians have a responsibility to
                   make clear and appropriate referrals to specialists, there is an
                   equally important responsibility on behalf of the specialist to

                   provide clear and timely feedback. Consider implementing the

                   following actions into your process:

                       •  Communicate your specific plan of action to the referring

                          physician to enhance continuity of care.

                       •  Include any medication changes, tests recommended

                          and performed, as well as the results and interpretation of
                          those tests.


                       •  If the patient refuses your advice or recommendations,
                          document this in the record and convey it to the referring

                          physician.
