Medical assistants (27 percent), registered nurses (23 percent),
and licensed practical nurses (11 percent) were most frequently
involved in the reported errors. Physicians (8 percent), nurse
practitioners (5 percent), and pharmacists (2 percent) were
involved less frequently, suggesting that most of the reported
errors are related to vaccine administration and not prescribing
or dispensing.
