Permit Systems and Entry Permits
Entry permits are an essential part of the overall permit-required confined space program. These
permits document and verify that entries are conducted properly.
A WRITTEN permit system must be in place to handle entry permit. Employers must have
a written confined space program if employees enter permit required confined spaces.
The entry permit must contain the following information:
1. The space to entered
2. The purpose of the entry
3. The date and duration of the permit
4. The names or other means of identifying authorized entrants
5. The names of personnel serving as attendants
6. The name of the person serving as supervisor
7. The hazards of the space to be entered
8. The measures use to isolate the space and to eliminate or control hazards
9. List the acceptable entry conditions for authorized entrants to safely enter a confined
space area
10. All test results with name or initials of tester(s)
11. The rescuer and emergency services to be used and a means for contacting them
12. An established communication system to be used between the attendant and the entrant
13. List of all equipment needed for entry and emergencies if necessary
14. Any other information pertinent to the entry
15. Any additional permits
