BEFORE entry into a permit-required confined space
                                                         is initiated, the entry supervisor must sign the entry
                                                         permit. This signature confirms that all safety
                                                         procedures have been  reviewed and  that entry is
                                                         permitted. The entry supervisor must be competent in
                                                         confined space entry policies and procedures.

                                                         The  WRITTEN  permit  must  be  posted  or  made
                                                         available to the entrants. This notifies entrants that all
                                                         pre-entry safety checks have been completed on all
                                                         hazards associated with the permit space.

               The main goal of the standard is to protect workers from the hazards associated with confined
               space. Permits must be issued only for the length of time needed to complete the job listed. When
               the assigned task is completed, the entry supervisor must cancel the permit by signing the permit
               to indicate that all operations are complete, and that equipment has been removed and placed
               into storage.

               Cancelled permits  must be kept for  at least one year.  Permits must  be retained  by the
               employer, and they should be used to assist with the annual permit space program review.
