Unless we determine and fix the most upstream causes of the
accidents, they will continue.
Avoid taking the easy route. Don’t place blame on the first cause that
comes to mind. Instead, keep asking “why?” to get to the most
upstream causes of the problem.
Injuries are a result of many contributing causes.
What is People-Based Safety (PBS)?

A proven science provided by Dr. Scott Geller that uses on Observation/Feedback
Process (OFP) to help prevent injuries by:
Observing work activity behaviors that are contributing causes to injuries (i.e.,
body mechanics, use of PPE, use of tools)
Providing immediate constructive feedback:
o Positively reinforcing safe behaviors
o Coaching to correct at-risk behaviors
Increasing employee involvement
Increasing ideas for process/behavior
Challenging workers to achieve 100% safe behaviors
