                                                    Cause & Effect

                 Key Points

 Unless we determine and fix the most upstream causes of the
                        accidents, they will continue.
 Avoid taking the easy route. Don’t place blame on the first cause that
comes to mind. Instead, keep asking “why?” to get to the most

                        upstream causes of the problem.
 Injuries are a result of many contributing causes.

                                      What is People-Based Safety (PBS)?

               ______________________________________________________________________

A proven science provided by Dr. Scott Geller that uses on Observation/Feedback
Process (OFP) to help prevent injuries by:

 Observing work activity behaviors that are contributing causes to injuries (i.e.,
body mechanics, use of PPE, use of tools)

                    Providing immediate constructive feedback:

                          o Positively reinforcing safe behaviors
o Coaching to correct at-risk behaviors

                    Increasing employee involvement
                    Increasing ideas for process/behavior

 Challenging workers to achieve 100% safe behaviors
