10.5 Worker’s Compensation Benefits

The Company is covered under statutory State Workers' Compensation Laws. If you sustain a
work-related injury, you must immediately notify your Supervisor. A list of approved facilities is
located on the Emergency Action Plan or by contacting Human Resources. In the case of an
emergency, you should go to the nearest hospital emergency room for treatment and then
contact your Supervisor or Human Resources for assistance.
               10.6 Unemployment Compensation Insurance

nou Systems contributes to the States and Federal Unemployment Insurance Funds on behalf of
the employee. Should your employment be terminated by nou Systems and provided that you
have met all qualifications for eligibility, then you may apply for and be eligible to receive
unemployment insurance benefits. Your voluntary separation from service for any reason,
termination for cause/disciplinary matters, or other reasons outlined in State Unemployment
Compensation Laws may preclude you from receiving full benefit payments.

               10.7 Medicare and Social Security
nou Systems deducts Social Security Insurance (for Old Age, Survivors, and Disability) from your
paycheck, as required by law. nou Systems deducts Medicare Basic Hospital Insurance from your
paycheck. These deductions are paid by both you and nou Systems.
