               Cafeteria Plan

               GMR offers a Cafeteria Plan that allows employees to pay for health premiums with tax-free

               payroll deductions and to contribute to a Medical Flexible Savings Account and/or a Dependent
               Care Account through tax-free payroll deductions.

               We maintain formal plan documents explaining the rules by which each benefit must operate,
               in accordance with IRS Section 125 regulations. These rules may be modified from time to time.

               To participate in each benefit, employees must make an election to authorize the tax-free
               deductions in exchange for certain restrictions. In addition, the money deducted may only be
               used to pay for eligible premium (employer provided benefits), medical or dependent care
               expenses.

               You will also be provided with a form when you are first eligible for health benefits to authorize

               GMR to deduct your health-related benefit premiums on a tax-free basis.

               Human Resources will provide you with forms to make a voluntary annual election to
               participate in the Medical Flexible Spending Account and/or the Dependent Care Account and
               to determine the dollar amount you wish to contribute each year. The elections will generally
               cover deductions for a full calendar year. However, you may begin making contributions when
               you are first eligible if you are not offered participation at the beginning of the calendar year.



               Income Replacement


               GMR provides income protection to full-time employees. We offer Short Term Disability
               Insurance, Long Term Disability Insurance and Basic Life Insurance at no cost to the employee.
               The Company pays the full premium. These benefits will replace a portion of your income in the
               event of death or disability.

               In addition, GMR provides the ability for employees to increase their life insurance amount
               through voluntary, employee paid benefits.

               In the event of your death while covered under the life policies, your chosen beneficiary will
               receive the death benefit. Contact Human Resources for a beneficiary designation form, which
               we recommend employees review and update at least annually.


               Human Resources will provide employees with a detailed summary explaining the benefits and
               exclusions under these group policies.
