1. ADMISSION REQUIREMENTS

a. Freshman Students

1. Report Card (Form138)/Permanent Record (Form 137A);

2. Certificate of Good Moral Character;

3. Photocopy of Senior High School Diploma;

4. University Admission Test Result;

5. Four (4) copies of 2x2 ID Picture;

6. Certificate of Physical/Medical Examinations;

7. PSA/NSO Authenticated copy of Birth Certificate; and

8. Other requirements prescribed by the College/Department,

CHED, PRC a deemed necessary.

b. Transferees from SUCs and Non SUCs must submit the

following:

1. Certification of Grades showing all the subjects taken from the school

last attended;

2. Honorable Dismissal;

3. Certificate of Good Moral Character

4. Four (4) copies 2x2 ID picture with white background and name tag

(scanned picture or digital copy not accepted);

5. Authenticated PSA copy of Birth Certificate;

6. Accomplished Substitution form of subjects taken from other SUCs

as deemed necessary; and
