What are Advance Directives?
An Advance Directive is a legal document that helps the patient’s doctors and family members understand his or her wishes about their healthcare needs.
It can help them decide about treatments if the patient is too ill to decide for him or herself.
You have the right to decide what medical care or treatment to accept, reject or discontinue. If you do not want to receive certain types of treatment or you wish to name someone to make your healthcare decisions, you have a right to make these desires known to your doctor, hospital or other healthcare providers and to have these rights respected.
Care is provided to all patients with
or without advance directives. When admitted, you will be told about advance directives and given written information
about them. Please do not hesitate to contact your nurse, physician or any other party for more information about advance directives.
Living Will
A Living Will provides instructions that specify what kind of treatment should be given to a person when that individual is unable to make decisions or speak for him or herself and is usually completed in advance of any known illness. It can be speci c or general.
Power of Attorney for Healthcare
A healthcare power of attorney allows a patient
to authorize another person to make medical decisions on his/her behalf when the patient is no longer able
to do so.
Advance Directive Forms
If you have an advance directive, please provide
us with a copy as soon as possible upon your admission to the Heart Hospital.
Insurance & Billing
Insurance
• Wewillneedacopyofyourinsurance card and any insurance forms that your employer or the insurance company requires. You will assign bene ts from the insurance company directly to the hospital.
• IfyouareamemberofanHMOor PPO, your plan may have special requirements, such as a second surgical opinion or pre-certi cation for certain tests or procedures.
• Youareresponsibleformeetingthe requirements of your plan. If your plan’s requirements are not followed, you may be  nancially responsible for all or part of the services rendered. Some physician specialists may not participate in your healthcare plan and their services may not be covered.
• Ifyouhavenoinsurance,arepresentative from the Finance O ce will discuss  nancial arrangements with you or assist you in applying for Medicare and Medicaid.
Medicare
Be aware that the Medicare program speci cally excludes payment for certain items and services, such as cosmetic surgery, some oral surgery procedures, personal comfort items and hearing evaluations, to name a few. Deductibles and co-payments are also the responsibility of the patient.
Medicaid
If you are covered by Medicaid, be aware that Medicaid has payment limitations on a number of services and items.
Your Hospital Bill
 e hospital is responsible for submitting bills to your insurance company and
will do everything possible to expedite your claim. Please remember that your insurance policy is a contract between you and your insurance company and you have the  nal responsibility for payment of your hospital bill. We have several payment options available to assist you in paying your bill. Patients can request an itemized statement by calling Patient Financial Services at 337-470-1311.
Additional Bills
You will receive a separate bill from your private physician and any consultants, radiologist, pathologist or anesthesiologist involved in your care, as they are independent contractors that bill for their services.  eir charges are not included in your hospital bill.
