HorizonBlue.comNeed Proof of Coverage...Fast?If you lose your ID card or need to visit a health care professional before you receive your ID card, you can now print a replica ID card. Through HorizonBlue.com, you may view and print a replica card that will serve as confirmation of your coverage.The electronic replica of your card features all of the information health care professionals need to know. You can carry thiscard with you and present it to your doctor%u2019s office or anywhere else your policy details are needed. To view and print a replica ID card, you must register as a subscriber for Member Online Services on HorizonBlue.com. To makeyour registration as quick and easy as possible, please have your member ID or Social Security Number available.Once you are registered for Member Online Services, visit HorizonBlue.com/members and click Sign In within the Member Online Services box. Once signed in, you can print your own replica ID card by clicking Print a Temporary ID Card within the left-hand navigation. Just follow these steps: Verify the name of the person for whom you are requesting the replica ID card. Select the coverage type (Medical, Dental or Prescription). Confirm your current mailing address. Select Print a Temporary ID Card/ Coverage Letter, and click Submit. You will then see an image of yourID card that can be printed and usedas proof of coverage.
