A 19 is the code to use when you will discharge
your discipline (not agency discharge)
For Medicare patients, when you plot a 19, plot the HHCCN buddy
code for the last day of your services.
A PT/OT/ST 18 is the service code to use when D/C
from VNSNY.
This buddy code should be plotted on the visit BEFORE the last
skilled visit for the cert period. This is remind the clinician to
deliver the Generic or NOMNC Medicare notice.
This notice is to be given to the patient at least 48 hours PRIOR to
the projected D/C date.
