1. You have a visit on your rolling calendar for that patient in the next 7 days.
2. You have seen this patient in HCHB in the last 7 days.
3. You are listed as a Case Manager/Team Member on that patient’s chart.
Steps to access Medical Records outside of a visit:
1. Click on your Menu button (3 dots) and then click on Medical Records:
2. You’ll find a list of patients that meet the above criteria. Once you click on that patient, you should be able to
access their information.
o History- viewing
o Features-doing (Use this section to add Coordination Notes, add New Orders to update Calendar/ freq,
address IDT pre-meeting work
How to write a New Order outside of a visit
o Menu>
o Medical records>
o Find the patient, click on any visit >
o Scroll to “features” section (or collapse “History” section>
o Click on “new order” >
o Enter all fields in the new order (send to physician, verbal order date, wound care orders: no/yes ABN: NO etc) >
o Complete the instructions text box with why you are writing the new order >
o Then swipe UP to reveal the gray buttons >
o Click on “add calendar” >
o Choose the day/s you want the service to begin (can use “start multiple select”)>
o Click “add/edit” service code”>
o Click “add”>
o Choose correct discipline for the service desired>
o Choose the service code needed>
o Press the back button to back out and return to calendar
View Attachments (Outside of the Visit)
To view your attachments:
Tap the device Menu icon from the 7-day rolling calendar.
• Tap the Medical Records.
• The Client List will display.
• Tap the desired client to expand his visit listings.
• The visit History menu will display
• Tap on Attachments.
• Available attachments will be listed on the bottom half of the screen.
• Tap and Hold on desired attachment listed to begin downloading and viewing the attachment
• Click the continue option.
• The Document will open for review
OASIS ALERTS for assessing clinicians after a completed SOC
Sometimes during the course of reviewing SOC or evaluation documentation, it is determined that additions or
modifications to the POC and/or the OASIS are needed. When edits are made to the OASIS, this function allows the
clinician responsible for the original entry to review requested changes/additions, and approve or disapprove the
request.
