Resumption of Care Process
For a patient returning home after being hospitalized, a Resumption of Care (ROC) visit (RN/PT15) is required.
If a patient resumes their care with VNSNY AND it is within the last five days of their cert period, then an 05 service code
is required.
• Intake associate write a coordination note “Resumption of Care” which goes to the CFM and is visible in
Pointcare.
• The CFM or the Clinician can write a new order, review and approve it, the scheduler plots and assigns an 05 to
the clinician.
• If the clinician is informed about the patient’s return to home, create a new order for the ROC:
o Under Medical Records, choose the patient you want to create a ROC order for.
o Select Features, then New Orders.
o Add a new order, with the order type Schedule ROC Visit.
o Complete the rest of the order and sync.
• IF the patient is in the hospital past the end of the cert period, the patient will be discharged from the agency.
Discharge your discipline (Service code 19)- HHCCN notice!
Planned discipline discharge
If you are able to project your discipline d/c date at the SOC, there should be a ‘19’ plotted to be the last discipline visit.
• Complete the ‘19’, issue the proper Medicare notices (if applicable), sync your visit and the CFM will
review your discipline discharge.
Early discipline discharge
From Outside of the visit (you know in advance that the next 11 should be a 19)
Write Scheduler Notification Coordination Note
- Outside of the visit, tap the 3 dots and select Medical Records.
- Select the Patient and tap Features, then Notes, then Add.
- Tap Type and select Scheduler Notification
- In the note request the discharge and the type of discharge.
From Inside the Visit
- Restart the visit, sync, call the scheduler, and ask for them to change the code to a ‘19’.
- Sync twice and you will be allotted the appropriate visit.
If you are unable to receive the appropriate code timely:
- Continue completing the 11 visit, then
- You must complete a coordination note, “Discharge Summary” for an early discipline discharge.
- Then enter a New Order via the features section of the medical records.
- For an early discipline discharge the clinician must write a new order to manually remove the remaining visits.
Discharging from the Agency (Service code 18)- NOMNC! (at least 48 hours
prior)
Planned
There should be an ‘18’ visit plotted on the calendar for the agency D/C visit. Clinician completes the 18 d/c visit, syncs
the visit to the office, CFM reviews documentation.
