c. Plot ONLY the evaluation visit on the calendar and attach “01” service code for the needed discipline
(PT01 for Physical Therapy; OT01 for Occupational Therapy; or ST01 for Speech Therapy).
• Subsequent visit:
a. Complete a New Order and choose “Physician Order” for the specific Rehab service.
b. Add reason for referral in “Instructions”
c. Add the ONLY the evaluation visit to the Calendar and attach the appropriate service code.
HHA Processes
HHA Initial Order at SOC
CLINICIAN adds HHA discipline from Need for Other Disciplines pathway
• enters HHA Schedule into Calendar
• enters HHA Tasks into Aide Care Plan
• enters Coordination note (HHA Order) to indicate HHA hours to primary and secondary payor (if applicable)
• completes SOC Assessment, signs off on visit note, and Syncs.
• calls Contract Admin to place HHA Order after Sync
• informs Contract Admin that it is an HCHB patient the following information:
1) Staff ID
2) Patient Name
3) HCHB MRN
CLINICIAN provides Contract Admin the Order information:
1) HHA Schedule
2) HHA End date
3) If Split Bill, number of hours billed to primary payor and secondary payor if applicable
4) Service Type (e.g., Mutual Care/Cluster Care)
5) At least 1 personal care task
6) Special Requests (e.g., pets in the homes, smoking, Kosher)
7) Primary Language
Other instructions (e.g., use side door to ring doorbell)
CLINICIAN ends call.
HHA Initial Order after SOC
CLINICIAN creates a physician order and enters the HHA Schedule and HHA tasks into AIDE Care Plan
• Enters Coordination note (HHA Order) to indicate HHA hours to primary and secondary payor (if applicable)
• Completes visit, signs off on visit note, and Syncs.
• Notifies CFM to obtain approval
• Calls Contract Admin to place HHA Order after Sync.
• Provides Contract Admin the Order Information:
o Staff ID and HCHB MRN
o HHA Schedule
o HHA end date
o If Split Bill, number of hours billed to primary payor and secondary payor if applicable
o Service Type (e.g., Mutual Care/Cluster Care)
o At least 1 personal care task
o Special Requests (e.g., pets in the homes, smoking, Kosher)
o Primary Language
o Other instructions (eg. Use side door to ring bell)
CLINICIAN ends call.
