Home Health Aide Care Plan
o If you need to order a HHA for the patient you MUST first choose “Need for other Disciplines” pathway
then choose Home Health Aide. You will need to enter an Aide care plan and the aide frequency to the
calendar.
o You will need to enter the aide frequency to the calendar AS WELL AS the Aide care plan.
Claim Codes
• In subsequent visits, click on “claim codes”; press save.
It is defaulted to “direct skilled care”. ONLY change if you have done something OTHER THAN direct skilled care.
The only time a rehab clinician would choose another claim code would be for a MAINTENANCE case.
PRN Section (from visit note main menu)
• Vaccinations
o There is no option for UK, if the patient says they had a vaccination and do not recall the exact day then
st
choose the 1 day of the month they had the vaccination
• Allergies
o Must enter allergies or indicate that there are no known allergies.
• Use the "Medication Understanding" section in the “PRN” section to document all medication teaching details
(specific meds) and response. Click on the medication you are teaching. Use “Med Understanding Notes” to indicate
who specifically was taught, if they were unable to demonstrate medication understanding and/or other details.
Sign Out Complete/Incomplete- get MUST get the patient’s signature before you leave the home
• Select incomplete if you need more time for documentation outside of the home. The visit will fall into the
overdue section after midnight
• Complete visits will disappear from the rolling calendar once you sync. (After you sync you can view your visit
documentation in Medical Record section)
Coordination Notes (from visit note main menu)
• To add a new note:
o Select “Notes”
o Select “Add”
o From the “Type” drop down menu, choose the appropriate Coordination Note type (See separate tip
sheet for note types)
o Depending on the note type selected, either a note template or blank text box will display.
o Once you enter/edit note text, click on the top right menu (three dots) and select “save”.
• To view an existing note:
o Select “Notes”
o Tap on a specific note read the content
*** If a coordination note is written while in a visit, that visit
MUST be completed AND Sync’d for other clinicians to be able to
see it.
