Search for the appropriate service codes. Important service codes to remember: PT11/ OT11/ ST11 for subsequent
visits; PT19/ OT19/ ST19 for DC of services, not from agency; PT18/ OT18/ ST18 for Discharge from agency.
Adding an Order -on Subsequent Visits
o Select the appropriate order type: Plan of Care Update, Lab Order, or Physician Order
o You must use Plan of Care Update to add Pathways. You must select this order type before any other
order type (if you select another order type first, you will not have the option to use Plan of Care
Update.) If entering Plan of Care Update (Pathway), you can include other orders (meds, visit freqs.) in
this order, except for Labs.
o The Lab order must be entered independently as this order is transmitted directly to Apex Labs.
o If updating visit frequencies and/or duration, use Physician Order.
