Client Meds (for SOC)

Visit Actions → New Order, on subsequent visits
o Indicate Order Date & Verbal Order Date and Time. Write in “Instructions” box a descriptor, such as “New
Medication order”. Then click on “Add Medications”
Visit Actions →Client Meds, if on admission
o Click “Add” to enter a new medication
o Search for drug. Use “Quick” for search of common drugs. Search “All” if not in quick list
o Select “Dose” from the dropdown
o Enter “Amount” eg., 2 tabs
o Enter “Frequency” from the dropdown selection.
o Enter “Alt. Route”
o Check “PRN” box if indicated
o FOR Subsequent visits, if a new medication is added, use “Instructions” to:
o Enter “Instructions” for patient label, if applicable.
TAPERED MEDS example:
If the following MD order is received: “Prednisone 40mg x 2 days, 20mg x 2 days, 10 mg x 2 days”
Three different med items need to be entered.
Prednisone Oral 40 mg Daily x 2 days, dates input will be 5/1/18-5/2/18
Prednisone Oral 20 mg Daily x 2 days, dates input will be 5/3/18-5/4/18
Prednisone Oral 10 mg Daily x 2 days, dates input will be 5/5/18-5/6/18

SLIDING SCALE example:
If the following MD order is received: “Novolog insulin 70/30, 25 units subcutaneously three times daily after meals.
If BG >200-250, administer additional 5 units
If BG 251-300, administer 10 additional units
If BG 301-350, administer 15 additional units
If BG >350, call MD”
Those amounts don’t need to be entered as individual meds. The sliding scale can be typed out into the “Instructions”
field.
You should always add a med from the data base as opposed to entering it manually, otherwise the DUR will not be
accurate

For Therapists, they will answer “Meds not reviewed” the CRM will review and speak with the patient if applicable.

• Plan of Care Supplies
o This is where you add the general category needed for the patient.
o If no supplies are needed, click “ No Supplies” and click “Save”
o If you need to indicate supplies needed or DME that is recommended or that the patient has, press “add”, in the
search box type your desired item, ie. Cane, press search and line items will populate below.
 Press and hold on desired line item, lift finger and the option to add the item will display. Press it and it will
be added to the supplies list.

o Locator 21 Orders: Here is where you are verifying POC orders for the patient.

• ICC or Integumentary Command Center
o You MUST Open this section even if there are no wounds. Click the word, “Document”
