 M2401 Intervention Synopsis: At the time of or at any time since
the most recent SOC/ROC assessment, were the following
interventions BOTH included in the physician-ordered plan of care
AND implemented

• Interventions to prevent pressure ulcers
• Pressure ulcer treatments based on the principles of moist wound
healing

M1306

M1306 Does this patient have at least one Unhealed Pressure Ulcer/Injury
at Stage 2 or Higher or designated as Unstageable? (Excludes Stage 1 pressure injuries
and all healed pressure ulcers/injuries)
Answer Yes if :
If there is a Stage 2 pressure ulcer
and/OR

Opened Stage 3 or 4 pressure ulcers
 And all unstageable pressure ulcers :
 d.1 non-removable dressing (e.g.NPWT)
 d.2 covered with slough or/and eschar
 d.3 suspected DTI
