Role of WOCN at VNSNY

 To provide clinical staff with guidance in development of an evidence- based
plan of care for patients with complex wounds, ostomies, urinary and or fecal
incontinence .
 Clinical consultations with WOCNs are provided in all VNSNY
Regions/Programs.
• Clinicians can generate a wound or ostomy consult request in
coordination notes using the note “Wound consult request”. WOCN will
request for wound photo if needed. (Photo process elaborated on later
slide)
• WOCNs review WAT score deviations and request for wound photo if
needed
 Joint home visit for complex wound and problematic ostomies.
 Provide continuous education- monthly orientation and skills labs for new
hires and incumbent nurses and developing educational newsletters
supported by the Education Development department.
 Review policies and procedures .
