Scenario 1
From RN Eni (ending shift) to RN Maya
I Patient details:
Retno Palupi, 38 years old, on Bed 8.
A frequent flyer and is under the treatment for thalassemia. Admitted in the morning,

for the complaint of dark urine for the last three days.
Feeling cold easily in a normal room temperature.
Diagnosed having Thalassemia 5 years ago. Issues are related to fatigue and trouble
B breathing, no history of dark urine before this.
Regular blood transfusion every four months, prescribed with vitamin and Deferasirox
taken one per day.
A Condition is fair with normal range of vital signs but with some discomforts. No chest
Ambulatory.
pain and no yellowish skin.
R
Recommends reticulocyte count for the consideration of iron chelation for her future
therapy.
