               Excerpt 1:

I am Risa, the nurse of the morning shift and now I am handing over the new patient to your care for the
following shift. Mr. Haryo Wibowo was admitted to the hospital this morning at 9 a.m. over concerns of a
head injury following a vehicular collision. Besides the concern of a concussion, his bleeding seems to be
heavier for the level of the injury he has.

dr. Purwo assessed him and overviewed his chart and confirmed that he has not suffered a concussion. The
patient has appeared alert, oriented, and also stable. Hence, he requested to be discharged. However, dr.
               Purwo recommended we continue
monitoring him for another day to
observe his bruises and bleeding. In
               case of swellings and on-going
               bleeding, MRI test is recommended
               to investigate possibility of
               Lymphoma.
