From RN Eni (ending shift) to RN Pitha
I Patient details:
Vitto Handoyo 24 years old, on Bed 11.
Admitted in the morning for the complaint of pain and stiffness around elbows for the

last three days with no sign of recovery. Stiffness deteriorated on day 3 and decided to
consult a doctor.
Fell from the stairs with minor cuts and bruises. Signs of bruises are visible even after
B three days. No observable external bleeding during the incident. No hospital visit,
wounds were treated by over-the-counter pain killer and ointment.
CT scan indicated signs of internal bleeding.
A Vital signs are still within normal limits but possible of sudden drops for indication of
Un-independent, loss of co-ordination and balance. Need bedside care.

R Request of Clotting Factor Test of Factor Assays to confirm indication of Hemophilia.
