1 Assisting a Library Customer to Fill Forms
a. Listen to an audio between Jack (Student) and Tom (Librarian) and complete the
following dialogue with the expressions you heard.
Jack : ”Excuse me, I'm a new student and I'd like to register as a library member. I heard I need
to fill out an e-form, but I'm not sure where to start.”
Tom : ”Hello, welcome to the library! You're correct, all new member registrations are done
online through our e-form. It's a simple process.”
Jack : ”Great. Where do I find the form?”
Tom : ”You'll find it on our main website. Go to the homepage and 1) _______ for the 'Services'
tab. From there, 1)_______ 'New Member Registration.' That will take you directly to the
form.”
Jack : ”Okay, 'Services' and then 'New Member Registration.' Got it. What kind of information
do I need to 3)_________ on the form?”
Tom : ”The form will ask you for some basic personal information, like your name, student ID
number, and your university email address. You will also be asked to 4)_______ a
password for your library account.”
Jack : ”And once I've filled everything out, what's the last step?”

page. Click that, and the system will process your application. You'll receive a
