ACCIDENT INVESTIGATIONS
All claims will be thoroughly investigated. The employee, the dispatcher, and Loss Control/Pre- vention Specialist (LCS/LPS) at PRO Resources Corporation all hold a key role in the investigation process. Theaccidentinvestigationprocessandtheresponsibilitiesspecifictothosekeyrolesare outlined below.
Employee/Driver Responsibilities:
1) Report to dispatcher ASAP.
2) Obtain appropriate medical attention as needed (if emergent, seek care first; then complete
following). IFSEEKINGMEDICALCARE;THEYMUSTHAVEPOSTACCIDENTDRUGTESTING. If they seek care later without an opportunity to complete timely drug testing, disciplinary action may be taken for failure to follow company policy.
3) Call PRO – Voicemail is available 24/7 at 218-847-9277 ext 219 or 1-800-776-4671 ext 219. Leave a message with the following details:
a) Name
b) Foltz Trucking
c) Day/date
d) Time and location of accident
4) Take Photos – Use Accident Kits provided in ALL trucks. •Takeaminimumoften(10)photosfromdifferentangles. Forexample,iftherewasaslip
or fall on the ice or over muddy ruts, photograph the larger area; photograph the spot from different view points.
• If possible and appropriate, photograph the injured body part. For example, if employee twisted his/her ankle, photograph the injured foot/ankle (with footwear and without
if possible)
5) If accident occurred on property outside of employer premises or current job site; report accident to the property owner. For example; if you slipped and fell at a truck stop, report your injury to the truck stop manager.
6) Write down details about the weather conditions on the accident report included in kit. Write down any other details that may have contributed to the incident.
Foltz Trucking Responsibilities:
1) Dispatcher: Upon receiving call/notification from driver of an incident; remind them if they plantoseekmedicalattention,post-accidentdrugtestingmustbedone. Iftheemployee seeks care later without giving the employer an opportunity to complete timely drug testing, disciplinary action may be taken due to failure to follow company policy and/or DOT regulations.
2) Dispatcher contacts Tom or Ken.
3) Tom/Ken contact PRO Resources Corporation after receiving word of an incident/injury.
Provideasmuchdetailasyouhaveavailableatthetime. Iftheonlyinformationyouknowis
nameanddate;callwiththosebasics. Don’tdelaywaitingformoreinformation!
4) Try to obtain the following details:
a) Thetimeemployeereportedtheinjury
b) Locationemployeecalledfrom
c) Whether or not he/she is seeking medical treatment and if so, where d) Request&obtainacopyofthedriverlogs;
