Obtain medical attention as needed (if emergent, seek care first; then complete the required phone calls). IFSEEKINGMEDICALCARE;YOUMUSTHAVEAPOSTACCIDENTDRUGTEST.
If you seek care later without an opportunity to complete a timely drug test, disciplinary action may be taken up to and including termination for failure to follow policy.
Report incidents/injuries to Dispatch/Tom. Leave a message on his voicemail.
Call PRO Resources Corporation – available 24/7 at 218-847-9277 ext 219 or1-800-776-4671ext219. Leaveamessagestating:
Name; Foltz Trucking; Date, Time and Location of accident
Take Photos – Use Accident Kits provided in ALL trucks.
• Takeatleast10photosfromdifferentangles. Forexample,iftherewasaslipontheiceor
over muddy ruts, photograph the larger area as well as the spot from different view points.
• Ifpossible(andappropriate),photographtheinjuredbodypart;i.e.,ifyoutwistedyour ankle, photograph the injured foot/ankle (with and without footwear)
If accident occurred on property outside of employer premises; report accident to the property owner. For example; if you slipped and fell at a truck stop, report your injury to the truck stop manager.
Write down details about the weather conditions on the accident report.
