FOLTZ TRUCKING CLAIMS MANAGEMENT
If you are injured, Foltz Trucking and PRO Resources Corporation work together to proactivly manageyourclaiminordertoexpediteyourrecovery. Claimsmanagementpivotsontimely sharing of information with appropriate parties involved with each claim.
Employee Responsibilities:
1) Provide medical report to Dispatcher, Ken or Tom at Foltz Trucking after initial appointment and after every subsequent medical appointment. (This can be by fax, email or phone call to the company with an update and return to work status.)
2) Participate in medical treatment plan – physical therapy, independent exercise, etc.
3) Participate in return to work options as part of your treatment/recovery plan. The first option is return to work options with Foltz Trucking. If that is not possible; then return to work options will be found near Foltz Trucking or close to where the employee lives.
Foltz Trucking Responsibilities:
1) Forward medical reports and/or information to PRO Resources Corporation in a timely manner.
2) Seriously consider return to work possibilities based on identified restrictions. Offer return to work options consistently to employees based on availability of work and specific physical limitations.
3) Keep PRO Resources advised when work is available and when employee does return to work. PRO Resources Corporation must also be notified immediately if employee does not show up for work, whether light duty or regular duty. PRO Resources Corporation also needs to be notified when the employee transitions from light duty to their regular job.
