Urine Specimen for Controlled Substances
Drug and Alcohol Policy
Drug testing shall be performed on urine samples and shall be tested for the presence of drugs or metabolites of the following controlled substances:
1. Marijuana 2. Cocaine
3. Opioids
a. Codeine b. Morphine
6-AM {heroin)
d. Hydrocodone
e. Hydromorphone f. Oxycodone
Oxymorphone
4. Phencyclidine (PCP)
5. Amphetamine
a. Amphetamine
b. Methamphetamine c. MOMA
d. MDA
Collectors who have received formal training shall perform collection of urine specimens. Urine specimens shall be tested at a Substance Abuse and Mental Health Services Association (SAMHSA) certified laboratory. Specimens shall undergo a screening test, and if necessary, a confirmation test.
Test results shall be reported by the laboratory to a Medical Review Officer (MRO) designated by Foltz Trucking Pursuant to DOT regulations, individual test results for drivers/applicants and drivers shall be released to Foltz Trucking and shall be kept strictly confidential unless consent for the release of the test results has been obtained. Any individual who has submitted to drug testing in compliance with this policy is entitled to receive the results of such testing on timely written request.
Training
Foltz Trucking shall ensure supervisors, designated to determine whether a driver should undergo testing for reasonable suspicion, to receive at least 60 minutes of training on alcohol misuse and at least
