4. Maintain confidentiality of critical information during the immediate recovery process by securing
the area from unauthorized persons.
Long-Term Recovery of Water-Damaged Paper Documents
1. Contact one of the following resources for document recovery:
a. Munter’s Corporation – Moisture Control Services

b. Belfor USA

2. An inventory of any items sent off-site for restoration will be maintained by the disaster response
team.
Document Reconstruction
1. To the extent that critical function information cannot be recovered by electronic data recovery or
paper document recovery, staff will attempt to reconstruct information by:
a. Reprinting or uploading documents from any undamaged databases
b. Obtaining copies from recipients of previously distributed copies, such as physicians’
offices, other healthcare facilities, or insurance companies
2. Make a record of the following, if possible:
a. Resident Name
b. Date
c. Type of information damaged/destroyed
d. How the information was damaged/destroyed
e. Signature of Executive Director
3. If unable to reconstruct any patient health information, a log will be maintained to document the
following:
a. The date of the disaster event and type of disaster that occurred
b. A description of recovery efforts, successes, and failures
c. A list of resident records effected
4. Damaged health records not able to be recovered will be destroyed in such a way that
confidentiality of the records is maintained.

1. The Executive Director, or designees, will attempt to manually restart the system.
2. If unable to log into the system, Practice Fusion support will be contacted.
