of the date they would like to deliver the items for their loved one. Patient Relations can be
contacted at each of our 3 sites as follows:

                    St. Joseph’s: 416-530-6652 or patientrelationsSJHC@unityhealth.to.
                    St. Michael’s: 416-864-5215 or patientrelationsSMH@unityhealth.to
                    Providence: 416-285-3666 ext. 4555 or patientrelationsPHC@unityhealth.to

Patient Relations will pre-screen the items the individual wants to bring in for the patient to
ensure they meet the outlined criteria. They will also make sure the individual bringing the items
to the hospital is aware that the items themselves must be wipeable or in a bag or container that
               is wipeable.

The Patient Relations team will schedule a time to meet the family member at the hospital front
entrance to pick up the items and deliver them to the patient’s care team in a patient belongings
bag. If items are being picked-up, such as laundry, Patient Relations will pick these items up
from the patient’s care team in a personal belongings bag and deliver them to the family
member at the front entrance as scheduled.

We will ensure items being dropped off from home are safe for the patient and staff by taking
               the following actions:

 When families or friends drop off items, the items will be screened at the door
 If the items do not pass the screening criteria they will not be accepted
 If the items pass the screening, they will be disinfected and placed into a clean patient
belongings bag before being delivered to the patient care unit

See Appendix A for disinfecting procedure

In addition to the above, deliveries from external vendors (i.e. flowers, gift baskets) will not be
               accepted for any patient.

               2. Delivery and Transfer of External Food for Patients
Patients are not permitted to bring food into hospital that is either commercially prepared or
prepared in a private home. This includes any type of pre-packaged snacks or food. The only
exception to this is if food is deemed to be culturally required and is not provided by Food
Services. If a patient requests a food due to cultural requirements staff are to contact the diet
office at their site as follows.

                    St. Joseph’s: ext. 6122
                    St. Michael’s: ext. 2794
                    Providence: ext. 4143 or 4142

If a patient is permitted to have a food item prepared in a private home based on the criteria
above the following process is to be followed.
