As a vascular surgery nurse practitioner, there are so many
Start Date: October 1, 2006 memories of collaborating with my wonderful

interprofessional team and vascular surgeons as we provide

care to our marginalized patient population-an incredible
team of people who work together to ensure our patients

are well-cared for in the hospital and while also ensuring

they transition safely into the community.

One of our patients required us to create incentives for him
to want to eat and to get out of bed and participate with

the team–our team used stickers, rewards, finding extra

moments to sit with him and reassure him, bringing him

special treats –all in an effort to support his return to his
group home. We collaborated with the community liaison,

advocated for and were successful in securing a more

accessible residence. I occasionally run into this patient in
the hospital lobby as he comes in for dialysis and marvel at

what our team accomplished in this case and in so many

others. I could not be prouder of these amazing health care

professionals and human beings!
