          NEW HOLTER MONITORING ECG CASES
  VetBooks.ir Question 58

Holter recording summary from a 6-year-old male, castrated Boxer with a history of ventricular premature
complexes (VPCs) and ventricular tachycardia (VT) on an in-hospital ECG, due to arrhythmogenic right
ventricular cardiomyopathy (ARVC). The summary displays the number of VPCs and runs of VT during the
24-hour recording period as well as the VPCs as a percentage of the total QRS complexes. Antiarrhythmic
therapy was started (sotalol 2 mg/kg q 12 h PO) to suppress the ventricular arrhythmias. A recheck Holter
recording was acquired after 1 week of sotalol therapy.

VPCs/24 h Runs of VT/24 h Ventricular ectopic beats as percentage of total
                                                          number of beats/24 h
            Pre-treatment 5,822 42 6.2%
            Sotalol 14,681 295 13.8%

1 What is the interpretation of the Holter recording results?
2 What recommendations regarding drug therapy might be made?
