30. Mounded Bullets.
a. All pumps and equipment cooling water outlets are connected to in adequate header and flow is restricted.
This lead to many leaks and stagnated. The header to be replaced with higher size and free flow to be ensured.
b. The damaged fencing and other approaches to the mounded bullet area has to be repaired and shall be 100%
protected zone.
Action: Maint- Field (Offsite) Target: 31 December -2021.

a. OM&S has to ensure the chain & Lock in the valves. Also they have to raise the notifications for damage of fire
proofing and non-functioning of gas detectors immediately and get it rectified.
Action: OM&S Target: 31 August -2021.
