Introducing Aflac Dental Coverage






        National Billing is pleased to offer Aflac Dental Coverage on a voluntary basis to all employees. This
        coverage is not dental insurance in the sense that you may be used to with networks and deductibles.
        This dental coverage, like other Aflac & Allstate Benefits coverage’s, is “Indemnity” coverage, meaning it
        pays a fixed amount, per covered dental service, based on the schedule of benefits in your policy, up to
        the policy annual limits. Aflac Dental has no networks, so you can go to any dentist you like, and you
        can have the benefits paid to your dentist or to yourself. Coverage is available for you, your spouse, and
        your children and you can keep the coverage for as long as you like, even if you change employers or
        retire.

        Aflac offers 3 Dental Plans:


        Aflac Dental Essentials:  is the lowest level plan and was designed to be used in conjunction with

        existing Dental Insurance. Dental Insurance often leaves policyholders with out-of-pocket expenses
        like deductibles and co-insurance – especially when you have major dental work done.  Aflac Dental
        Essentials was designed to help with those out-of-pocket dental expenses as it pays in addition to any
        other dental insurance you may have. Again, there are no networks - you can go to any dentist anywhere
        in the country!  Aflac Dental Essentials is the only Aflac dental plan that can be used with existing
        dental insurance.


        Aflac Dental Plans 2 & 3: These plans are available as stand-alone dental plans and pay higher
        benefits than Aflac Dental Essentials.  Again, you are free to go to any dentist you like as there are no
        networks.  You can provide your dental plan information to your dentist and have them submit the
        claims, or you can do it yourself and have the claim payments sent directly to you.  All three plans cover
        the same services with the difference being the amounts each plan pays. Essentials pays the lowest,
        with Plan 2 paying more, and Plan 3 paying the most. These plans may not be used with existing dental
        insurance.

        How It Works



        Aflac Dental Essentials: You use your regular dental insurance as you always do. Your dentist
        submits the dental claim to your primary dental insurer, and they pay your dentist whatever they pay, and
        you are billed for the remainder.  At the time of your dental visit, on your way out, ask for a copy of the
        claim they will be submitting.  You upload a copy to the Aflac Claims Website or App, and you have the
        option to have Aflac pay you, or your dentist whatever your Aflac Dental policy pays for the covered
        services you received.  As this is not primary Dental Insurance, you do not list it as dental insurance.

        Aflac Dental Plans 2 & 3: These can work more like traditional dental insurance.  You can give your
        Aflac Dental Plan information to your dentist, and they will submit the claims directly to Aflac and be paid
        directly (usually much quicker than traditional insurers pay). Like any other dental insurance, you may be
        responsible for any amounts not covered by your Aflac Dental Plan.  Remember, Aflac Dental is not part
